How does it work? It’s easy.

  • Our Care Management Help Line is on duty Monday through Friday from 7:00 am to 10:00 pm and Saturday from 8:00 am to 5:00 pm, Central Time
  • Get the best deal on your prescriptions
          - We’ll give you price quotes on all your medications
          - Our pharmacists will answer your medication related questions
  • We’ll help you complete all the forms over the phone and place your order

Or, place your order through the mail or via fax. Follow these simple instructions:

1. Download and complete the Order Form

  • Click on the appropriate form image to download and print the Order Form
  • Complete the entire form, and be sure to include all current medications you are taking, any allergies you have and your payment information
  • Please remember to sign the form
  • MedVantx accepts all major credit cards
  • You can also pay by check. If paying by check, please call us at 1-888-825-8474 to make sure you have the most current price for the medication you are requesting.

2. Fax or mail the completed Order Form

  • Once you have completed the Order Form you can either fax to 1-888-825-8473 or mail to

  • If you are paying by check, you must mail the Order Form along with your check

3. Provide MedVantx with your prescriptions

  • You can either mail your prescriptions with your Order Form, or we can call your doctor to obtain your prescriptions
  • Faxed prescriptions must come from your physician’s office
  • You cannot directly fax us your prescriptions

How long does it take?

Your prescription will generally arrive within 3-5 business days of when MedVantx has received your completed Order Form, valid prescription and confirmed payment. Your order will be shipped via the US postal service and arrive in a secure, private package. Remember, MedVantx offers FREE standard shipping on all orders.
